Nutrition and Hydration Resource Pack for Care Homes (Brighton and Hove area)

This resource pack has been made available by the Community Dietitians in the Brighton and Hove area and has had input from the MDT including SLT.

The Nutrition and Hydration Resource Pack for Care Homes is designed to promote excellence in nutrition and hydration care in a care home setting. It links with local guidelines and supports local care pathways for the identification and treatment of malnutrition, through the use of MUST and Food as Treatment.

It contains record charts that can be adapted for use by individual care homes for use in care planning for individual residents and detailed information (and recipes) for chefs about increasing daily calorie and protein intakes.

It also includes sections on Hydration, Mouth Care, Swallowing Issues, Dementia, Dysphagia and End of Life considerations in nutrition and hydration care. The use of this resource pack is supported by the community dietitians in Brighton and Hove and High Weald, Lewes and Havens.
