Practices who are part of West Wakefield Health and Wellbeing are using social prescribing to connect patients to non-medical sources of help. This can address issues such as unhealthy lifestyles and social isolation that are the root of many visits to the GP.

In West Wakefield, older patients are referred to navigators who help them identify local activities or groups that would interest them. This video helps to explain the approach to patients.

Healthy London Partnership have just published a video for patients to explain what social prescribing is. This is the kind of thing that could be used in a practice waiting room or linked on the website, to help maximise patient engagement in a social prescribing service.
