Hampshire Healthier Together partnership for children and young people

Specialists, primary care teams and others have come together across Hampshire to provide innovative new services for children and young people. The Healthier Together initiative aims to improve health services and to build families' confidence in looking after themselves.


Online resources provide information and support for parents and healthcare professionals. These have been developed collaboratively between primary and secondary care leads.

The programme has now also launched Paediatric hubs, based around the local primary care networks serving 30-50,000 patients. This allows primary care professionals to book patients directly into a monthly clinic run by a paediatrician and GP, which also provides an open access multidisciplinary case discussion meeting for any professional wanting to discuss a case. This new collaborative service is funded through the Sustainability and Transformation Partnership.
