Collaboration between GPs and social workers in Larwood and Bawtry Primary Care Home

Two GP practices serving villages in Nottinghamshire and South
Yorkshire have come together to form a Primary Care Home (a type of primary care network). One of their priorities was to find better ways of supporting patients contacting the practice about issues relating to housing and social care.

The network have created a weekly social care clinic hosted in one of the practices. Here, patients can see a social worker easily. It is also easier for GPs and social workers to get to know one another personally, discuss particular issues and consider alternate approaches to meeting needs locally.

The clinic is part of wider work including the use of social prescribing and input from the primary care home to local work to improve housing conditions.

The impact so far has included a significant reduction in the time it takes for patients to engage in social care solutions. Patients are seen within one week now. GPs report they feel more empowered to address their patients' needs in the most appropriate way.
