West Wakefield Health & Wellbeing is a social enterprise established to support primary care transformation, initially through the Prime Minister's GP Access Fund and later as part of the new care models vanguard programme. 

GP appointments are in demand more than ever before. As doctors, we know that patients often present with complex needs that require support from a range of professionals and this can be difficult to address within a short ten-minute consultation.

Care navigation is a simple, sustainable model of care that improves access to primary care and reduces GP pressures. By providing clear information about the range of services available both inside and outside of the GP practice, care navigation provides real choice to patients allowing them to go straight to the service that best meets their health and social care needs.

West Wakefield Health & Wellbeing has successfully developed and implemented a highly effective care navigation model that could be replicated to transform primary care and community services in your area. From April to August 2016, it helped to signpost over 9,500 patients and saved over 1,145 hours in GP time across Wakefield.

Training for active signposting

57 receptionists in 6 practices across West Wakefield have been trained to work as care navigators for patients. Working with Certa, our national awarding organisation partner, we have created a unique Accredited Certificate in Care Navigation training package.
