Innovative new diabetes care being tested in Bradford

Multi-agency approach to diabetes care in Bradford including involvement of the voluntary sector.

 

Local clinical leaders have joined forces with other organisations to develop an innovative new way of managing diabetes, which is being tested at several GP surgeries in Bradford.
Living Well with Diabetes is a partnership project between local NHS Clinical Commissioning Groups (CCGs), the Royal National Institute for the Blind (RNIB) and Public Health at Bradford Council. The initiative uses specially designed folders to help patients with diabetes keep track of their medication, appointments and their own actions to help improve their health.
