About us


To lead the implementation of the North Mersey pathway of diabetes care, providing timely, accessible, evidence based care and support to patients with diabetes by suitably qualified members of the health community that fulfils national (DNSF, NICE etc) and local standards and patient expectations.


• To lead the health community by example in providing evidence based guidelines and support tools to facilitate continuous improvement in practice and to support the measurement within constituent organisations of the improvements in clinical outcomes and patient experience.

• To provide a scheduled clinician led programme of work that will support the education of the health community that supports patient empowerment.

• Work across boundaries to improve patient experience and clinical outcomes that produce system wide benefits.

• Produce a strategy to be used as a benchmark tool for all aspects of care that will advise commissioners of the key priorities in the short, medium and long term.


The network has a core team including a network manager and lead jointly by a primary and a secondary care clinician (currently a GPWSI and consultant Diabetologist) and chaired by a PCT CEO with a remit to set the strategic direction, provide leadership and be the central point of contact and communication; supported by clinical representatives from the multidisciplinary team to develop specific projects within the programme of work and act as the communication conduit to their organisation and wider network professional group.
