Electronic Palliative Care Coordination System

 

Key Points:

As part of NHS Somerset’s End of Life Strategy, an Electronic Palliative Care Coordination System (EPaCCS) has been designed to enable healthcare professionals across the community to record and share information relating to palliative care patients, including preferred place of care, details of carers, diagnosis, resuscitation preferences and other key information.

 

Anticipated Benefits:

Enables patients to choose their place of death and have that choice communicated across the health community 

It means their wishes are respected, their dignity preserved and unnecessary and unwanted admissions to hospital are avoided, if at all possible and appropriate.
