Clinical and Service and Integration - The Route to Improved Outcomes
This paper by the Kings Fund (first published 2010) describes and summarises relevant evidence about high-profile integrated systems and outlines examples of integrated care for particular care groups or people with the same diseases or conditions. It also reviews ways of achieving closer integration for individual service users and carers through care co-ordination and other approaches. The paper is aimed primarily at policy-makers and NHS leaders working on these issues, in the hope that the evidence brought together here will help to inform the future direction of reform.
