Train for Trainers Programme for Self Care & Shared Decision Making

With support from the Department of Health, Dr Alf Collins on behalf of the South West SHA, has designed a two day training for trainers programme in shared decision making, self management support and care planning. The programme aims to develop a cohort of trainers that have the knowledge, skills and confidence to support organisations in their area to reliably and systematically deliver self care support and shared decision making. Participants will be able to provide training for a variety of services including • community Matrons, re-ablement teams • specialists in Long Term Conditions • Interface services • Primary care teams • Secondary care teams The training takes participants through a range of modules for use with teams/services at various levels of activation. Throughout the two days participants are given opportunities to practice and develop their confidence in facilitation and coaching skills. The lead facilitator for the programme is Dr Alf Collins, National Clinical Lead of the Health Foundations Co-creating Health programme, with support from other experienced facilitators including experts by experience. The powerpoint in combination with the workbook, provide participants with a range of flexible sessions and exercises that can be delivered to teams and services, a suggested structure for these sessions is provided, which can be adapted to suit almost any service.
