As part of the Improving Rehabilitation Services Community of Practice NHSCSI supported providers in raising the profile of local service improvements taking place or being developed. NHSCSI creates a collaborative environment where innovation and learning can be spread to inspire sustainable change.

The following snapshots (organised by region) represent the updated, second collation that includes snapshots added between January and the end of February 2015. where the authors have given their consent for the sharing of the information and of their contact details for follow-up enquiries.

Participants continue to send-in snapshots and these will be added to this collation in updated versions of the document to follow.  Each collation is shared with the Improving Rehabilitation Services Community of Practice, the NHS England Improving Rehabilitation Services programme team and disseminated through professional bodies.
