What happens when people leave hospital and other care settings?
When discharge goes wrong, it comes at a cost, both to individuals and to the health and social care system, according to a report from Healthwatch.
The number of beds lost as a result of discharge delays has risen from 4,754 in July 2015 to 5,861 in July 2017, a rise of 23.3%. In order to further understand people’s experiences of leaving care in light of these increases, Healthwatch has brought together a summary of the experiences of more than 2,000 people have shared with 46 local Healthwatch organisations during the last two years.
