Project to cut hospital admissions wins Sir Peter Carr partnership award

A CCG has won this year’s prestigious Sir Peter Carr partnership award for their innovative project which prevents avoidable hospital admissions and provides care at the place of a person’s choice.

Somerset CCG, Yeovil District Hospital and South West Academic Health Science Network are behind the project, which encourages nursing homes, general practices, out of hours and emergency services to implement the National Early Warning Score (NEWS2). This creates a common language across these services which enables safe, effective and timely patient care. The CCG will be awarded £30,000 which they will spend on building team knowledge and skills.
