Primary care home community of practice seeks members

The primary care home programme is a joint National Association of Primary Care (NAPC) and NHS Confederation programme that aims to develop and spread NAPC's primary care home (PCH) model.

The PCH is also featured in the recently published multi-speciality community provider (MCP) emerging care model and contract framework as a model for delivering the MCP care hub or neighbourhood approach, aimed at redesigning care around the health and wellbeing needs of the local registered population.

A letter of invitation  provides further information regarding the community of practice and how your site and aspirant PCH networks can apply to join.

Please share this letter with any interested PCH networks within your CCG. Completed applications should be returned to napc@napc.co.uk by Friday 21 October.
