Patient and family centred cultures: building the evidence base for outcomes
Designers of value-based care systems face a challenge in making decisions that are both right for whole communities but also tailored to the needs of individuals.
Commissioners and providers need to work with patients, carers and professionals to discover the priorities that matter most to them. This includes defining the right outcomes for the affected communities (or populations).
These must be meaningful, measurable, explicit and based on robust evidence. Being seen to focus on outcomes will help your organisation build support, withstand scrutiny, manage change and weather financial and other challenges. The right outcomes also give systems permission to stop delivering care that is not making a difference in patients’ lives.
