NHS Institute take initiative to join up care
“Patients might see up to 10 separate professions and be asked the same questions again”... “They are dealing with fewer people and there is a seamless transfer of information”...
This is just one before and after experience taken from a recent case study – the latter being the result of adopting a joined-up care approach, which the NHS Institute has now developed into a suite of products to help eliminate the duplication, inefficiency and waste which can create a poor experience as patients pass between organisations on their healthcare journey.
The NHS Institute’s Joined-up Care initiative has identified those areas that will make the bigger difference, and has integrated these into three new resources which can help you to deliver seamless care:
