Implementation toolkit for local systems published

Good organisation of care between general practice and secondary care is crucial in making the best use of clinical time and NHS resources in both settings and in ensuring that patients receive high-quality care.

New measures were introduced in the 2017-19 NHS standard contract to improve working practices and patient experience between primary and secondary care. A toolkit including case studies supports local systems to work  together to implement these changes. NHS England claims the contract measures could save 225,000 GP appointments.
