GP visits key to care home’s success in cutting emergency admissions
A Nottinghamshire care home has cut residents’ emergency admissions by 23% and A&E attendances by 29% by creating a more effective care team across local provider organisations.
The home receives an enhanced care package from Principia Partners in Health, part of the national vanguard programme, in which local GPs work with Age UK, community nurses and care home staff.
