Tackling health inequalities in the homeless population in Southend

A new initiative spearheaded by local GP surgeries in a primary care network (PCN) in Southend-on-Sea, has seen groups of clinicians working together in collaboration with partners to deliver personalised care to the local homeless population.

Partners in the initiative include soup kitchens, hostels, outreach teams, the local hospital, council, mental health and substance misuse services. This example of integrated care has enabled the maximisation of opportunities to improve health.
