SocialPrescriberPlus – essential training for social prescribing link workers, care coordinators and health coaches

The SocialPrescriberPlus Programme is online training for primary care networks (PCNs) and practice social prescribing teams. With the additional roles reimbursement scheme now supporting social prescribing link workers, care coordinators and health coaches, practically focused training is essential for these posts to provide effective and efficient support to practices. This online training delivered via the Zoom meeting platform, runs across three half-day modules, with the first two modules taking place in the same week and the third module taking place after the social prescriber has gained experience in the role.
SocialPrescriberPlus is accredited by the Personalised Care Institute as providing training of the highest standard.

Module 1 – The role of the social prescriber in the PCN and practice (09.30-12.30) This module focuses on the soft skills needed by the link worker to engage effectively with the key stakeholders in the PCN and the practice, with their patients, their colleagues and with the community groups and services to whom they will be referring their patients. The module also covers telephone consultation techniques, building trust and rapport, and personalised health and care planning.

Module 2 – Case management skills with the patient (13.30-16.30) Module two builds on the innate human attributes of the link worker such as empathy and compassion and provides the essential skills needed to engage effectively and therapeutically with their patients. Using a personalised care approach and a structured meeting and conversation model the module covers the essential competencies of active listening, motivational interviewing and health coaching.

Module 3 – Reflective practice, active learning and peer supported networking (09.30-12.30) Module three takes place several weeks after the first two modules ideally after the link worker has started their role and has built up experience. In a facilitated learning environment, the link worker comes together with colleagues in a reflective practice and active learning session. The sharing of case studies and challenges in a facilitated and supportive environment builds strong peer supported professional networks that provide support going forward.

At the end of the programme, delegates will be fully equipped with the professional knowledge, skills, expertise and confidence to make an immediate impact as a social prescriber or link worker in primary care.

The SocialPrescriberPlus Programme is led by Nick Sharples. Nick is a highly experienced facilitator and consultant, and hugely knowledgeable about the current primary care and social prescribing environment. He has delivered training to new and prospective social prescribers and to large social prescribing schemes throughout the UK and Ireland.
