Primary care is the gateway to health services. It has a pivotal role to play in co-ordinating the patient’s journey towards improved health and well-being; it must ensure the elderly, the sick and vulnerable are appropriately cared for and prevent well people becoming sick. The vanguard and Prime Minister Challenge fund sites, through collaborative working are testing new ways to deliver services in primary care more efficiently and effectively to achieve better value for money and improved outcome for patients.


Their challenge is immense but not insurmountable – these test beds are proving change is possible with resolve and good leadership. This event will showcase the progress of some of those providers towards reforming primary care, widening its remit and reshaping how services are delivered.


Each speaker will be interviewed in depth following their presentation - probing questions asked, designed to elicit guidance for the successful development of new care models envisaged in the five year forward view. Delegates will benefit from advice based on their experience and lessons learned. They will gain a good understanding of the importance of stakeholder engagement and the fundamental importance of creating a shared vision that meets everyone’s expectations of a health service fit for the future.


The event will focus on the determinants for success. Delegates will hear of the ambitions of these organisations, the challenges they have faced and overcome; their achievements to date and their goals.
