Would you like your patient participation groups to work together across your primary care network to extend your links with local communities and support your social prescribing link worker?

Would you like local people to become health champions to support care navigation and help promote self-care?

Would you like your patient groups to be more inclusive and better represent your local population?

Would you like to understand how to work more effectively with the communities in your neighbourhood?

This workshop for PCN involvement leads, link workers and practice managers will reconsider the role of PPGs in PCNs and other involvement options for local people to support health and wellbeing in communities.

It will:

  • Increase your understanding of why working with individuals who use your services and the community is important, particularly in the changing NHS
  • Encourage you to think about how to work differently with people and communities and extend the reach of your social prescribing link worker
  • Give you ideas about how to develop your patient participation groups so that they can work together and become champions of good health
  • Increase understanding about working with people as partners in their own health.

The day will draw on your experience, use case studies of good practice and allow space for planning the way forward for your practice or primary care network.
