House of Care Legacy Event - Care and Support Planning for Long Term Conditions

We are pleased to announce the British Heart Foundation House of Care Legacy Event - Care and Support Planning for Long Term Conditions at York Racecourse on Tuesday 9th October 2018.

Thanks to advances in medicine, research and public health, people are living longer often with one or more long-term conditions (LTCs), including cardiovascular conditions. The traditional care model, reliant on episodic instances of care, often in acute settings, does not routinely provide the holistic care that best supports people living with LTCs. A new way of delivering person-centred care for people with long-term heart and circulatory conditions has been piloted by BHF. Care and support planning (CSP) using the House of Care approach as a framework sees healthcare professionals and people with LTCs come together as equal partners. Central to this is a collaborative conversation between patient and healthcare professional to discuss what matters to the individual and work out the care and support from which they might benefit, addressing both health and social care needs.

Join an audience of commissioners and planners from around the UK to hear their experience of adopting and implementing care and support planning the framework and learn how the key principles can be applied in your own area, for the benefit of both you and your patients.

You will hear more about key recommendations from the programme, including:

  • The importance of clinical leadership in each practice and at a healthcare community level
  • Involving people with LTCs in the development and implementation of CSP
  • Equipping the workforce with ongoing training
  • Including the whole practice team
  • Allowing time for building the roof of the house, IT and processes
  • Flexing the framework to meet local needs
  • Improving and commissioning of social prescribing and more than medicine is crucial

Speakers include representatives from the Person Centred Care Group at NHS England, the Health and Social Care Alliance Scotland, National Voices and practitioners who have implemented CSP as part of the BHF House of Care Programme.


The event will run from 10.30am – 4.30pm and lunch will be provided.
