NHS Network 3 Counties Cancer Network
The 3CCN is a cancer network serving the populations of Gloucestershire, Herefordshire, South Worcestershire and parts of Powys. Although the Network has a relatively small population, it covers a large geographical area with patient pathways extending throughout the counties and to Birmingham and Bristol. Our core purpose is to act as advisors to commissioners, but we also aim to continually deliver improvements in cancer and palliative care for patients and their carers.

NHS Network Common Assessment Framework for Adults Learning Network
Documenting the learning from a three year programme to test different approaches to sharing information across agencies to support personalisation. JOIN THIS NETWORK TO GET UPDATES WHENEVER THERE IS NEW CONTENT. There are three main areas to this network. 1 Lessons Learnt from the CAF Programme (these can be found under three categories down the left 1. Redesigning Systems and Services, 2. Techincal Approaches, 3. General Programme Working and Support) 2. Message Board for ongoing networking, specifically in relation to information sharing across health and social care 3. Archived material from the old CAF Network used during the programme (again this can be found down the left side) The CAF Programme tested different approaches to sharing information across a variety of agencies to support the implementation of personalisation. There was an emphasis on maximising individuals’ control over their care and support arrangements (including over the information held about them by care and support agencies), and on the importance of the co-production of services and support: this presented many challenges to the established way of doing things. The three year CAF Programme started with a public consultation. Local Authority-led partnerships were asked to put forward an Expression of Interest to the Department of Health which resulted in nine partnership sites being selected in the first phase which started in March 2009. A further three sites were selected 12 months later. One site completed its work in March 2011, and the remaining 11 on 31 March 2012. A full independent evaluation report on the national CAF Programme will be available by autumn 2012. The sites were: Barnsley, Camden, Hampshire, Lincolnshire, Rochdale, Shropshire, Stockport, Warwickshire, Westminster as phase one sites and Cheshire East, Consortium (Hampshire, Portsmouth and Southampton) and Isle of White as phase two sites.
