NHS Network QIPP Digital Technology and Vision
The QIPP Digital Technology and Vision network is to promote and provide an online collaboration space for the sharing of local digital initiatives across the NHS. The Digital Technology team works closely with local and national teams through the QIPP Workstreams and SHA CIOs to stretch their ambition and understand their priorities. On-going central support is required to help teams deliver their initiatives at scale and pace, reduce duplication and ultimately enable their QIPP plans. We are a small team and only focus on areas where there is strong demand from the NHS and clear benefits that can be derived from the use of digital technology to enable the delivery of QIPP plans for areas such as Urgent Care, Long Terms Conditions and End of Life. We believe that the national role is to support local development and accelerate delivery of digital services through the creation of a number of national enablers including: information standards; interoperability specifications; technical guidance including deployment patterns; and knowledge sharing to accelerate the pace of spread. As part of the knowledge sharing enabler we have been working with representatives across all SHAs to understand the current digital initiatives that have been delivered or in progress linked back to the QIPP work-streams. More details can be found in the Digital Activity Map section of this site. A copy of the ways of working document that we have developed with the Long Term Conditions work stream is included as an example. To date we have engaged with and have an on-going dialogues with the following national workstreams: Urgent Care Long Term Conditions End of life and the National end of life programme Rightcare Safecare Procurement

NHS Network Long Term Conditions (LTC) Electronic Templates Supporting Personalised Care Planning
The Long Term Conditions Programme within NHS Yorkshire and the Humber's Programme for IT was established with the aim to improve the experience of patients with Long Term Conditions. Working largely with existing systems and technology, the programme included the provision of templates to support personalised care planning. Electronic templates within SystmOne were developed in partnerhip with clinicians from across the region for a number of long term conditions. These templates can be found in an exportable format on this website, along with data mappings for those that wish to recreate the templates within alternative IT systems.
