NHS Network Long Term Conditions (LTC) Electronic Templates Supporting Personalised Care Planning
The Long Term Conditions Programme within NHS Yorkshire and the Humber's Programme for IT was established with the aim to improve the experience of patients with Long Term Conditions. Working largely with existing systems and technology, the programme included the provision of templates to support personalised care planning.
