Improving outcomes in cardiovascular disease
A guide for practice-based commissioners
This guidance provides PBC groups with a framework for investing funds to improve outcomes in cardiovascular disease. It reviews the extensive evidence base for promoting prevention and optimising treatment in CVD. It then outlines how PBC groups can use savings to fund a more systematic approach to the management of CVD in primary care.
The framework focuses on increasing case-finding in CVD risk, hypertension, atrial fibrillation and diabetes, improving the manage-ment of hypertension, angina and diabetes, and targeting preventive interventions on those at high CVD risk. Cheshire and Merseyside Cardiac Network and the Heart of Mersey cardiovascular disease prevention charity have worked closely with the ChaMPs practice-based commissioning project team to develop this paper.
