The Long Term Conditions Team in the Department of Health is interested in any current tools that have been developed or are in the process of development that health and social care professionals can use to support people with long term conditions to change behaviours, in particular tools that use coaching and motivational interviewing techniques that are specifically tailored for people with LTCs.
