A lot of work has been undertaken (or is being planned) across West Yorkshire, on care pathway mapping and redesign, to improve the services we provide to patients. In order to share the work more widely and avoid duplication and re-inventing the wheel each time, it was thought useful to have an overview of this work, which could then be shared across West Yorkshire.

A standard pro-forma has been used to collect core information in a structured manner, this information is in a summary format, with contact details of the lead clinician and manager to allow further information to be shared with interested parties.

The pro-forma has been designed to allow communities to identify work that is at different stages of progress (from planned to implementation), to facilitate early engagement and sharing of ideas.

The pro-forma is available on line and can be used by organisations to update previously submitted care pathways as work is being progressed, contact details updated etc. or to share with colleagues new care pathway work that is being planned or in progress.

All care pathways submitted must be signed off by the lead clinician and lead manager.  We would urge all communities to contribute to the establishment and maintenance of this facility in order to promote service improvement, share best practice and improve services for patients across West Yorkshire.
