Worcestershire Diabetes Network
Aim: To provide a structure for the service redesign, planning and delivery of diabetes services in Worcestershire.
The network will:

Engage all stakeholders, including clinical staff, managers, commissioners and people with diabetes.

Work across service boundaries in the whole health economy of Worcestershire.

Have clear lines of accountability.
-
Support staff to achieve agreed changes to models of care and care pathways of diabetes care.

Plan and advise on commissioning of diabetes services.
-
Take on the role of the 'Diabetes working group' of the Worcestershire care planning partnership.

Monitor quality of care.

Monitor standards of training across the county.

Invite appropriate persons to give regular updates on development and provision of diabetes services'.
All work co-ordinated and driven by the network will aim to ensure:

Care is delivered as locally and effectively to patients homes as appropriate.

Diabetes care is integrated and seamless.
-
Patients are active participants in their care.

Structured education and support for self care is offered to every patient.
-
Communication with diabetes service is easy for patients and clinicians.
-
Implementation of the National Service Framework for diabetes and NICE guidance to improve standards of diabetes care, as well as ensuring achievement of national diabetes performance targets and indicators.

Equitable access across the network for patients to receive the best quality of care.
