Trafford Pharmacy Network
A joint network of secondary care, primary care and community pharmacists in Trafford.
This network has been formed to develop actions and solutions arising from Medicines Management issues that cross the secondary to primary interface care. Community Pharmacists are now actively included in discharge procedures.
The network is currently focusing on procedures for in-patient assessments for monitored dosage systems or other suitable adjustments and seamless handover of care to the community pharmacist.
Regular assessments by the community pharmacist and linked working with social carers will faciliate monitoring of community based high risk patients.
