A new Integrated Care Pathways Manual 2006 which the LTHT Performance Improvement Team has recently developed is now available. This manual replaces the 'ICP Toolkit' published in 2003.

The new manual is an NHS web based guide to leading and developing integrated care pathways and pulls together a variety of recommended tools and techniques and approaches to developing ICPs. It is an amalgamation of information and guidance from a number of recognised sources including the NHS Improvement Leaders Guides and National Electronic Library for Health (NEHL).

The manual has been designed for Health and Social Care professionals leading the development of Integrated Care Pathways across organisations. It  describes the full range of steps and processes that ICP project leads or groups will need to consider for successful development and implementation.

The Manual is intended for use by staff with some previous experience and knowledge of Service Improvement tools and techniques, and is closely aligned to the LTHT service Improvement toolkit, however individual sections of the manual may be helpful for members of project teams working on individual components of the development work and these chapters can be accessed at the click of a mouse on Leeds Health Pathways http://nww.lhp.leedsth.nhs.uk/icp/
