Information A virtual ward is a cadre for providing support to people with the most complex medical and social needs. The ward uses the systems and staffing of a hospital ward, but without the physical building: it provides preventative care for people in their own homes. Admission to a virtual ward is determined solely by the output of a predictive risk model (such as the PARR tool or the King's Fund/Health Dialog Phase III algorithm). This forum, currently operating through yahoogroups, is for sharing ideas between professionals across the UK who are interested in establishing virtual wards in their areas. The group is open to Community Matrons, GPs, PCT Managers, Social Workers, Allied Health Professionals and Public Health practitioners - as well as to interested parties from local and central government, the voluntary sector and the private sector.Members are reminded to maintain professionalism in all postings, and must never post any information which contains patient-identifiable data. Documents and presentations saved in the 'files' section may be reproduced and shared openly. However it is a condition of membership that the original author and his/her organisation must always be acknowledged. To join the group or for more information, please contact Dr Geraint Lewis via the e-mail below. To learn more about virtual wards, see the attached document. |
