Is anyone currently using PARR++? if so at what segment would you recommend to select patients from? I am part of virtual ward pilot, and unsure as to what pecentage to select patients from?

Hi, I'm one of 4 GPS in a Virtual Ward pilot in Wandsworth and we have the same question!
I've found that if you look back to far e.g. 6months over 3 yrs, you get high risk scores butt he patients have actually begun to stabilise already. If you look back over too short a period, you get very few numbers! Discussion with kings Fund led me to this forum - I was hoping I'd finad all our answers here! Any other ideas? |

Hi,
we've been using PARR++ for just over a year to identify patients to refer to a team of 30 community matrons. I base the risk segment on the capacity of the team. A risk of 70% or higher generally gives about 100 patients a month, roughly 3 new patients per community matron. Not the most scientific approach, but it works. |
