Default West Sussex Health Trainers

Any information anyone willing to share on what paperwork is in use in your organisation and how easy you find it to use. I would be interested to know how you collect the relevant information whilst retaining a client centred focus.

Hi All

Have found the site, now we can confer about related items and problems.

A problem shared is a problem halved.

I have every intention of making my own short check list for obtaining information. This way (hopefully) it will not appear to the client that I am just filling in a form.
I can then bring up questions, relating to the information required, in the conversation, filling in the form later.

I think a lot of basic information can be gathered on the initial telephone contact.

Default Paperwork Used

Hi

Here in Blackpool, we use the standard paperwork from the database. We complete this on the initial visit to the client. We advise the client that we have a lot of forms to complete, but it will be the only time we need the information & they have all been fine about it. We do this after explaining our role & checking that the client really does want to sign up to the service. I complete as much information as I can before the visit from the referral paperwork & the rest only takes about 10 minutes during the visit.

I recently completed the basic training for health trainer.I would like to know how you get your clients and how many have you had at any one time.

Forging links with community groups and initiatives. Referrals from practice nurses; email all the practice nurses in your locality or drop in personally to see them and explain what you do and give them your contact details. Arrange drop ins and get involved as a team or as an individual in local events.

I think the most one-to-one clients I have had on my database at one time, during my 18 months as a HT is c. 40! Far too many really; 30 is our recommended upper limit.
