For Community Mental Health Nurse band 5, I quite like to ask

'you visit a client at their home address. When you arrive the door is open, you knock and hear a shout to go in. You enter and find the client sitting in the lounge. There is a packet of tablets on the coffee table - you can not see but believe it to be empty; there is an empty bottle of vodka on the floor next to her feet and a knife next to her on the sofa. How would you proceed?'
