Under the practice based commissioning programme in East Devon, Claremont Practice were one of the first practices to grasp PBC and explore some of the opportunities it afforded. The practice started with well planned in-house peer review sessions led by the lead PBC partner. All GP partners, Practice Manager and other clinicians and practitioners within the practice met regularly to discuss referrals made and explore opportunities for managing patients more locally.

One of the first initiatives to be piloted was to develop their existing integrated team approach, funded under the PCT Aspirational Funding Stream , as an ‘Invest to Save’ Initiative. The original pilot aimed to provide additional Nurse and Social Worker hours in order to:

  • Reduce the number of emergency admissions to secondary care.
  • Identify patients likely to be at risk of an inappropriate hospital admission.
  • 'Case manage’ at risk patients – health and social care
  • Develop a ‘live’ Long Term Conditions register for patients at risk of admission to support proactive management.
