Maryport Group Practice Integrated Community Hospital PMS

The ‘PMS plus' pilot has developed integrated services with the community hospital (adjacent to the practice). Social services are based on site enabling support to the practice and other patients in the hospital or community. The social worker attends a weekly multi-disciplinary team meeting.
Maryport Health Services Cumbria also share an integrated IT record on all patients with community staff and social workers. This provides improved communication & easy access to better information for co-ordination and access to care. Source:
