Bristol Promoting Independence Team

This joint team offers a programme of prevention and support initiatives to support social inclusion and independence in the community for people over 65 years.
The focus is upon people whose safety and independence may be compromised by issues such as recent bereavement or trauma, difficulties in medication or mobility, housing or social isolation, demanding care commitments, or low level mental health needs.
This covers people who do not meet other Social Services Fair Access to Care criteria or who do not need rapid intervention services and are not receiving a service at the time.
The team encourages referrals from a range of sources including General Practice and works with clients for 6-8 weeks identifying statutory voluntary and community support networks including family that will assist in maintaining long term independence.
