Palliative Care Heart Failure patients are now able to access the same level of end of life care as cancer patients. This includes the patient’s choice of what setting they would like to be cared for and where they would like to die. This can be a hospital setting, hospice setting, at home or other residential setting.
Discussions around heart failure end of life care can take part at any point during the patient’s care, from diagnosis of heart failure to when palliative care needs have been identified. End of life discussions, planning and agreement take place between the patient, their families/carers and heart failure specialists and can be over a period of time to be able to offer exactly what the patient would prefer as their final choice of place of care and death.
