We offer choice in care options, respecting the need of being cared for as close as home as possible, without being stigmatised and promoting and enabling recovery and well being. We hence offer home and community based care.
Church End Medical Centre offers in house psychological and counselling services. Patients requiring these services book to see the clinical psychologist or the counsellor at the surgery. Being seen in the surgery takes away the stigma of "being referred to the psychiatric hospital". The psychologist and counsellor have direct access to the GP as all work within the same team in the same building. We have a Practice Nurse specialising in Mental Health. She provides holistic personalised care plans for our patients-she attends to their mental health condition as well as their concurrent long term conditions such as diabetes, hypertension or COPD. She works closely with the Care Co-Ordinator, who co-ordinates the health and social support services for the patients and their carers, hence offering early intervention services,preventing hospital admissions and improving the quality of life for the patients and carers. The Nurse has a good working relationship with the clinicians at the local Mental Health Trust as she was previously employed by the Mental Health Trust for many years. She helps to reduce outpatient DNAs, emergency admissions and non compliance with medication. When patients decline hospitalisation, she maintains their stay at home with the support of the GP and the Mental Health Trust Team. When they need hospitalisation, she can fast track their admissions. She has regular clinics at the surgery and does home visits. Categories for this entry: Choice and Mental Health |
