The aim of the End Of Life Care Initiative is to enable patients to die in their own home through:-
• Communicating & documenting patients/families wishes at the end of life.
• Providing skills & confidence for clinical staff to initiate those difficult conversations, through support & education.

Palliative care in a learning disabilities care home.
Following a period education & reflection, the nursing team are now enabled with the skills & confidence to initiate difficult questions with family, especially those discussions around resuscitation.

Gold Standards Framework in care homes (GSFCH)
A Worcester care home, who recently completed the GSFCH has introduced monthly meeting for staff & local GP`s to review & evaluate care of all residence at the end of life. Use of anticipatory prescribing has improved, DNAR documentation has been introduced & communication with out of hours via the Adastra system is becoming routine.

End Of Life Care mental health
The development of pain management tool for non-cancer, this is a flow chart system to assess & manage symptoms during last days of life; therefore preventing an unnecessary inappropriate admission into an acute hospital.
