Draft Models for Mapping Skills and Roles

 

The aim of the Workforce Change in Long Term Conditions programme has been to facilitate change in the design of local services and workforce and implement new ways of working. Through workshops we have worked together to learn and share relevant tools, techniques and information.  One of the outcomes from a recent workshop has been the development of diagrammatical web-based tools that could be used by local communities to identify the skills required for their future workforce in Long Term Conditions. These models are work in progress and are only suggested templates. Our seven SHA sites will continue to rework and define them in more detail over the next few months.

This 3D Sphere shows a non exhaustive skills list for people working in long term conditions, arranged over the Kaiser triangle. The red bands indicate the type of skill and the blue bands the actual skill.
The base of the triangle, level 1, represents the 70-80 per cent of the long term conditions population who mainly self care.  The middle section of the triangle, level 2, represents high risk patients, who need disease care management.  Level 3, the top part of the triangle, represents the small percentage of patients with highly complex needs, who require case management.
