National Primary Care Development Team (NPDT)

The National Primary Care Development Team (NPDT) is regarded as being at the forefront of improving management of long-term conditions in the UK, helping frontline clinicians and managers to deliver improvements in the way long-term conditions are managed at a local level.  The primary vehicle for delivering improvements in care is the National Primary Care Collaborative Phase III, alternatively known as the Chronic Disease Management Collaborative, a programme that close to 90% of Primary Care Trusts across England are involved with at present.

The Programme focuses on what are traditionally perceived to be the three levels of targeted care – self-care, disease management and case management in the form of Unique Care
