Case Management Competences: Proactive Management of Complex Long Term Conditions

The aim of this domain is to ensure the community matron or case manager is competent in enabling people with multiple long term conditions make informed choices regarding they own plan of care. The community matron or case manager will be able to identify and support the risks associated with caring for people with complex problems in their own home.
